Precision Care

Conquering the cognitive impact of infectious disease.

The precision care team includes faculty in family medicine, neurology, medical imaging, and psychiatry working together to provide advanced care for patients with cognitive impairments and to establish a clinic for long-COVID patients to assess cognitive impairments.

This team has extensive experience on epidemiology of dementia, and infectious diseases including HIV and SARS-CoV-2 (Saverio Stranges); primary health care, with a focus on patient-centred care and health care delivery (Bridget Ryan); and patient engagement, use of electronic medical records, and patient-oriented research in primary health care (Amanda Terry), infectious disease diagnosis and treatment (Michael Silverman), and mental health impacts of infectious disease on local First Nations communities (Arlene MacDougall).

This team is implementing Cambridge Brain Sciences cognitive screening tests in public health/family practice clinics, is developing referral processes for patients to long-COVID clinics for advanced care, and is facilitating partnerships with clinics in the Oneida First Nations, Southwestern Ontario. They are also establishing cohorts that will be participants in the clinical trials led by the. More specifically they are:

  • Using the Southwestern Ontario Health Hub platform to understand long-term impacts of COVID-19 and the pandemic on the Southwestern Ontario population by collecting and analyzing: population-level indicators on physical and mental health including cognitive function, climate, employment and socio-demography; primary care practice electronic medical record data; ICES health administrative data, and longitudinal cohort survey data to track changes in population physical, cognitive, and mental health over time
  • Conducting pragmatic trials focused on patient primary care and community public health interventions to improve health outcomes including for people with cognitive impairment. These will be co-created with patients, providers and community stakeholders and act at the intersections of multiple triggers including the pathogen, the individual’s health status, and the social determinants of health.
  • Engaging in knowledge mobilization that supports primary care practice screening, case finding, management, and treatments arising from VICI phase II trials for early COVID-19 triggered cognitive impairment.
